ACCIDENTAL DEATH & DISMEMBERMENT



 If death, dismemberment or loss of sight, speech or hearing should occur because of injury effected solely through accidental means and within one year after the accident, payment will be made to you if living, otherwise to your beneficiary if living, or to your estate as follows:

Both hands or both feet

Both arms or legs

One arm or leg

Sight in both eyes

Sight of one eye

One hand or one foot

Speech or hearing in both ears

Speech and hearing in both ears

Hearing in one ear

Thumb and index finger of one hand

Four fingers of one hand

All toes of one foot

Total and irreversible paralysis of: the arms and legs (quadriplegia): the legs (paraplegia): or one arm and one leg on the same side of the body (hemiplegia) 		  Percentage of Amount Insured ($50,000)
