PROOF OF AGE/DISABILITY
Before receiving a pension benefit a member shall be required to submit proof of age to the Trustees. If a member's pension is to continue after his death to a contingent annuitant, proof of age of the contingent annuitant shall also be required by the Trustees.
If you are applying for additional disability credits, you must also supply proof of total disability satisfactory to the Trustees.
