ANNUAL STATEMENTS
After the end of each Plan year, you will receive a statement showing:
. the number of hours reported on your behalf and the pension earned in the Plan year,
. the total monthly pension that you have earned in all Plan years, and
. your total credited service under the Plan.
This information will be considered correct unless you inform the Trustees in writing, with documentation of any discrepancies.
Periods of disability during a year may count, within limits, toward Future Service credits for the year. However, you must apply for such credits within 60 days of receiving your annual statement for the year.
Note: It is important to keep your pay slips because errors can occur in reporting or tabulating your hours. It is easier to correct such errors if you keep a record of:

[iii] the hours you were employed each month.
