APPLICATION FOR BENEFITS
No benefit is payable under the Plan unless and until a written application for such benefit has been filed with the Trustees. It is the responsibility of each member or other persons to whom a benefit is payable under the Plan to apply to the Trustees for such benefit.
Please notify the Plan administrator who will provide you with the necessary forms.
